Arrived at Soesterberg on 9 November 1974. Official delivery date according to the AFHRA records is 8 November 1974.

The aircraft was first assigned to the 401st TFW at Torrejon AB. It was leased to the Imperial Iranian Air Force from September 1972 until January 1973.

Left 32nd on 6 April 1976 to the 35th TFW at George AFB. Upgraded to F-4G in July 1979.

Delivered to 90th TFS/3rd TFW at Clark AB, Philippines, on 30 July 30 1979.

As F-4G active in September 1995 with 190th FS from the Idaho ANG at Boise.

To AMARC as FP1024 on 25 March 1996.

To Tracor, Mojave, CA. on 14 February 1997. Converted to QF-4G drone AF151.

To Holloman AFB. Expended in missile test 5 February 1998.
