Arrived at Soesterberg on 29 November 1974 from the 50th TFW at Hahn AB, Germany. Official delivery date according to the AFHRA records is 28 November 1974.

Left 32nd on 31 May 1976 to Hahn AB.

Went to the 163rd TFS of the Indiana ANG.

In May 1988 to the 110th Tactical Fighter Squadron of the Missouri ANG.

In 1990 to the US Army Development Test Area.

To Turkish Air Force in May 1991. Converted to F-4E-2020.

Crashed near Ayisigi, Turkey on March 17, 2009, shortly after takeoff from Konya AB. Both crew ejected but pilot was killed.
